What is Transitional Care?

The Transitional Care Unit (TCU) plays a vital role within the healthcare system by bridging the gap between hospital care and home or long-term care.

 

Designed for patients who no longer require acute hospital care but are not yet ready to return to their regular living environment, TCUs provide short-term, interdisciplinary support!

Benefits of the TCU

Individual’s needs will be met in a safe, comfortable inclusive setting, including but not limited to:

  • Providing caregiver relief and respite
  • A personal plan of care
  • Activity programs
  • Education, with goals to increase or maintain maximum health and well-being of everyone

What to Expect at The TCU?

Who is the TCU Team?

Our Care Team includes:

  1. Nursing Team: Registered Nurses (RNs), Registered Practical Nurses (RPNs)

  2. Nurse Practitioner

  3. Personal Support Worker (PSWs)​

  4. Health Care Aide (HCA)

  5. Dietary Aides

  6. Environmental Service Aides (ESA)

All rooms have a standing shower that can accommodate a shower chair.

What is Permitted?

Our transitional care unit features semi-furnished, hospital-style private rooms.

What We Do

Healthcare services, assistance with activities of daily living, social programming, virtual activities and more! (As per need basis)
Private Rooms: Rooms offer privacy; however, some may include an additional bed to meet personal or regional needs.
The company hosts events for caregivers, such as nursing parties, pizza parties, and Christmas celebrations.

What is Not Permitted?

  • Pet Visits: Pets are welcome with prior approval, provided they are healthy, vaccinated, and kept on a leash.

  • Personal Items: Clients can bring comfort items such as blankets, pillows, photos, and personal electronics (e.g., phones, tablets, TVs)

  • Decorations: Small, non-permanent decorations like family photos and artwork are allowed. Please avoid items that may pose safety risks, such as candles.

  • Smoking: Designated outdoor areas are available for clients who wish to smoke

  • Alcohol

  • Toiletries: Powder products (e.g., talcum powder) are prohibited due to safety concerns.

  • Scents: Please avoid using fragrances, scented products, or flowers to maintain a comfortable environment.

  • Money: Clients should carry minimal cash. TCU is not responsible for any money kept by clients.

  • Zero Tolerance for Abuse: Aggressive or abusive behavior, including physical, emotional, or verbal mistreatment, is not tolerated.

Family Involvement

Client & Family Responsibilities:

  • Transportation: Arrange transportation for admission, discharge, and medical appointments.​

  • Personal Preparation: Ensure residents are prepared with personal hygiene items, and incontinence supplies

  • Laundry Services: Coordinate weekly personal laundry or opt for laundry services provided by the TCU.​ Please aim to label all clothing and items to prevent loss during individual laundering.

  • Medication Costs: Cover pharmacy costs, including deductibles, dispensing fees, or co-pays for medications or treatments not covered by OHIP.​

What kind of Equipment Can I Bring?

  • Specialized Medical Equipment: CPAP machines, oxygen concentrators, mechanical lift slings as needed, or other home-use devices (with instructions).

  • Mobility Aids: Wheelchairs, walkers, canes, commodes, or shower chairs, raised toilet seats (all rooms have standing showers).

  • Furniture: Small personal furniture suitable for semi-furnished rooms.

  • Phone: Bring a personal phone or device with a data plan. Wall phone connections are available, Wi-Fi is provided.

  • Television: Cable is available for free; bring your own TV. Clients are responsible for setup and use.

Our team will assess all items upon arrival to ensure they meet safety standards.

Visitation & Contacts

Visiting Hours

Daily: 9 a.m. – 8 p.m.

Extended Visits:

For visits outside these hours, please speak with the nurse or administration. We’ll accommodate requests while ensuring client comfort and safety.

Visitation Guidelines

  • Keep Groups Small

  • Be Mindful of Noise

  • Follow Health Protocols: Adhere to health and safety measures, such as hand hygiene and mask-wearing, when required, to protect clients and staff

If you have concerns during your loved one’s stay, please reach out promptly. We're here to ensure a positive experience for both you and your loved one.
